I can't help but think that it must be a lymphoproliferative disease once you have their schemas (maybe a little biast). However, if he doesn't improve on corticosteroids and continues to have high SPAP, do you think a cardiac catheterization could bring us closer to a diagnosis?

My granddaughter, 4, has been having jerky movements for about a year, maybe be longer. She has bitten her tongue in her sleep and while awake, poked herself in the eye with a straw when drinking a juice box, fallen down all because of the s movements. She is on a wait to see a paediatric c neurologist, who knows how long that will take in Canada. Anyway it’s very concerning, thoughts?

How can gait be normal if the patient has a weakness of MRC 1/5 on the right side ? 2. How do we differentiate pachy- vs lepto- meningeal enhancement on imaging ?

Amazing humbling case

No one can figure out what is wrong with my much loved 53 year old husband. Fever going on 3 weeks that we know of. Knee and leg pain intermittent. High heart rate. Used to be 60s. Now often 100 to 119 even resting. Some vitals improved since put of 2day hospital stay. 26.8 crp, 1600s ferritin. Some other odd labs. Catscans, ultrasounds, mri normal. Neurology consult, infectious disease consult, rheumatology consult (possible Stills disease but symptoms don't quite fit). No rashes. Petscan today. I wish I could have these dudes analyze everything. Or doctor House. Too bad he's not real! They tell us cancer unlikely. Also negative for leukemia and lymphoma. Rheumatologist wanted to start on Prednisone already. I told him shouldn't we wait till petscan or clear diagnosis? He then agreed. Also, all docs think cancer isn't real likely but need to be sure. Also not meningitis or endocarditis. More odd labs but hard to include here. He has stumped the doctors so far.

No history of syncope or chest pain with that tight valve.
